Vaso-Vagal Response during bowel movement?

Probably 4 - 6 times /yr when I am having a bowel movement I get light headed, dry heaves, break out in a sweat and get very warm and weak. I lie down, chill, cover up and fall asleep and the rest of the day I am fairly weak. I had bladder repair surgery 2 years ago and probably within the year I have noticed my stools are pushing into my vaginal wall. I also don't have to strain very hard to have the sensation I mentioned. Is this a vaso-vagal response and should I be concerned?
