After years of not having a vv, mine was triggered a little bit after going on medication for diabetes (newly diagnosed). I was also dehydrated probably. My episodes have often been triggered by severe abdominal pain as well.

I don't know whether this is associated with this or not but after I did lose over 30 pounds by counting carbs for diabetes and I have not had a severe problem . I pay a ttention to my body and drink water when I need to, don't get up suddenly, etc. Hopefully I have averted episodes.
