4 stitches on my forehead

i'm a 37 year old asian male and believe ive had vasovagal syncope since i was a kid, although my doctors confirmed this only a year ago. my attacks are triggered solely by trips to the bathroom, more specifically when i take a dump. all of my attacks start with a terrible stomach ache(like having diaarhea)and a feeling that i could not get my feces to come out(like being constipated). i know it sounds contradictory but thats the way it feels. i almost always end up fainting, about 90% of the time. last year was the worst attack i had because i hit my head on the bathroom floor when i passed out and ended up having to go to the hospital to have my forehead stitched to close the gash.all sorts of tests were done(including the tilt table test)and my doctor recommended daily medication. i declined this since i only have attacks about twice or thrice a year, and it wouldnt be practical to have daily medication. recently my attacks seem to be increasing. in the last 3 months, ive already had 4 episodes and am really quite concerned already.what do you think i should do? my doctor recommends another series of tests but i think that its not really necessary anymore since they already know what my condition is. hoping that someone with more experience in this matter can help me.
